All clients must print and fill out #1 - Primary, #2 - Intake Questions, and #3 - Engagement Letter and include these with your other tax preparation documents you provide us - W-2's, 1099's, 1095 (for health care), etc..

When filling out #2 - Intake Questions, where you answer YES to any line, refer to the far right column IF YES, TAKE ACTION and follow those instructions.

If the ACTION refers to ORGANIZER [name], return to this page for that ORGANIZER FORM, print and fill out the form and include it with your submitted packet to us.
